Militaires Sans Frontières (French for Soldiers Without Borders; lit. Military Without Frontiers), abbreviated as MSF and also known as Outer Heaven, are the main protagonists of Metal Gear Solid: Peace Walker and Metal Gear Solid V: Ground Zeroes and the overarching protagonists in Metal Gear Solid V: the Phantom Pain.
It was a private military company in the early 1970s founded and led by Big Boss. Because of its mercenary status, questionable tactics on the battlefield, and usage of an off-shore forwarding operating base in the middle of the Caribbean Sea during the early-to-mid 1970s, it was frequently referred to by its enemies as pirates. The company was later succeeded by Diamond Dogs after its fall.

Foundation[]
After Snake abandoned the Patriots, he founded MSF to provide military force to anyone who needed it, regardless of nation or ideology. To avoid attention from rivals like the CIA, MSF would have no fixed abode. While working for the Colombian government, MSF battled a mercenary group led by Kazuhira Miller. Snake would recruit Miller, and he would become it's second in command.
Metal Gear Solid: Peace Walker[]
On November 4, 1974, while stationed in Colombia, Militaires Sans Frontières were approached by Ramón Gálvez Mena and Paz Ortega Andrade, to request aid for Costa Rica, as a mysterious security company had recently invaded their country, presumably for the CIA. In return, Gálvez offered the group an offshore plant to use as a base off the coast of the Caribbean. MSF commenced operations against Peace Sentinel, who were attempting to develop an unmanned nuclear weapon known as Peace Walker.
During their mission, the MSF also dispatched several of its forces into other war zones for income, in order to increase its numbers and resources (namely small arms, missiles and uniforms) as well as to feed its personnel, in the fight against Peace Sentinel. In addition, after capturing territory from the enemy forces, the MSF temporarily used them as training grounds for their troops in various weapons and fighting styles. When their leader, Hot Coldman intended to launch a nuclear strike on Mother Base MSF considered evacuating, but Miller had some confidence that Snake would succeed in stopping Peace Walker's launch. Eventually, MSF aided Snake in stopping the Peace Sentinel, who had relocated to neighboring Nicaragua.

After Mother Base's destruction, the only survivors of MSF present were Big Boss, Miller, and their chopper's medic. While Big Boss fell into a coma, and an artificial one was induced in the medic by Cipher, the vengeful Miller was left physically unscathed. The MSF members away from Mother Base at the time believed Big Boss dead, and went their separate ways. A few of them slowly went insane over the years, and were left wandering battlefields. During the early 1980's Miller formed Diamond Dogs as a successor organisation with some surviving remnants of MSF. After Big Boss's seeming return as Venom Snake in 1984, several former MSF members eagerly re-joined him, and Snake would also bring in the wandering MSF survivors.

- The name Militaires Sans Frontieres is a nod to the humanitarian organization Médecins Sans Frontières, which assists countries ravaged by war and epidemics that are in need of doctors and medical experts, with the fictional group being the military equivalent.
- In the Japanese novelisations of Peace Walker and MGSV, as well as the Japanese subtitles of The Phantom Pain, MSF is renamed Out Of Order.
- MSF is an early version of "Outer Heaven", Big Boss's concept of a nation of soldiers. While Outer Heaven ends up a terrorist organisation, due to Big Boss's descent into villany, MSF is more well intentioned.
