Dr. Rose Cotter is the main protagonist of the 2022 psychological supernatural horror film Smile.
She was a psychotherapist who was haunted by a malevolent and mysterious creature who feeds on the trauma of other people and then terrorizes them, tortures them, takes on the forms of their victims' fellow human beings and then possesses them and kills them in front of a witness to continue the suicide chain.
She was portrayed by Sosie Bacon.

Rose Cotter experienced a traumatic childhood due to her mother, whom suffered from mental illness and abused her daughter. One day, Rose's mother attempted to take her own life by overdosing on pills, but survived the attempt and reached out to Rose to call for help. However, Rose left her mother to die, resulting in her being guilt-ridden for years. Despite her actions, everyone believed that Rose's mother died from suicide, including Rose's sister Holly. As an adult, Rose started to see a therapist named Madeline Northcott before becoming a therapist herself. Additionally, she dated a police detective named Joel before becoming engaged to a man named Trevor.
Smile (2022)[]
Speaking with a woman named Laura Weaver, the patient explains that after witnessing her history professor take his own life, she has been terrorized by an entity that only she can see. During the discussion, Laura falls back and screams, sending Rose to call for help before seeing Weaver smile eerily and take her own life. After the event, the entity transfers itself onto Rose, tormenting Cotter by making taking the form a patient named Carl and telling her she is going to die. Rose calls for the nurses to restrain Carl, but returns to see him asleep in bed, leading to her boss Dr. Desai to give her a week off.
The entity continues to torment Rose, leading to Cotter to speak with her former therapist and be seen as unstable by her fiancé, sister and peers. She finds solace with Joel, who helps her investigate Laura's professor, and meets with a man named Robert Talley, who explains that the only way to get rid of the curse is to brutally kill someone in front of another. After leaving Robert once he learned that she is cursed, Cotter is confronted by the entity when it takes the form of her therapist. She then plans on passing the curse onto Dr. Desai, but awakes from the hallucination, unable to go through her plan.
Realizing that the entity can't pass onto someone else if she is alone, Roes returns to her old home and confronts it. Though it initially appears that Rose defeats the entity, it's revealed that her victory was another hallucination by the monstrosity. The entity then possesses Rose after revealing its true form, and has Cotter take her own life in front of Joel, thus passing the curse onto him.
Smile 2 (2024)[]
After Rose's death, she is posthumously mentioned in the sequel, as the entity came after her ex-boyfriend Joel, as he was seeing the Entity as a hallucination of her burning hersself, but after their talk with Talley earlier, Joel managed to rid the curse the same way he did by trying to pass it on to one of the heartless criminals but accidentally killed one but had it passed onto a drug dealer Lewis. Joel flees from the gang but gets accidentally killed by an oncoming pickup truck.
Later in the film as the Entity haunts the new protagonist, Skye Riley, a famous pop star. She visits a stranger named Morris who knows about the entity's existence after it killed his brother and has tracked it and briefly researched it ever since. As he mentions Rose again as a "doctor" who got it and all the other past victims who got it too before her, inculding his brother. Feeling remorseful for not listening. He followed up on what his brother said and found out his witness victim and victim who saw him die all said the exact same thing before him, Rose, Joel and Lewis did before all died.

- A deleted scene included Joel is first seen talking to Rose, trying to coversate her about her mental problems lately. He tries to convince her that she will be alright, and if there's any problem she can contact him. He hugs her as they both embrace one another, before she returns to her car.
